Rhinoplasty (nose job) surgery encompasses the following procedures:

The nose is the centerpiece of the face and as such has an important aesthetic value. The idea that there is one nose that fits everyone is myopic. A surgical or non-surgical rhinoplasty should be performed in harmony with the patient’s existing facial contours and ethnicity to create a natural and timeless result.

In addition to the aesthetic value of rhinoplasty, patients commonly seek nose surgery to alleviate long-standing breathing problems, mostly related to prior trauma. This often results in poor sleep for the patient as well as their partner (snoring). I will analyze your nasal airway during the consultation and discuss with you the improvement to be expected following surgery.

The Rhinoplasty Procedure

Surgical and non-surgical rhinoplasties are performed in harmony with your existing facial features to achieve a natural, timeless result. The most important part of a rhinoplasty is the consultation. During this time, your visualized result comes to life as we devise and align on a plan, surgical or non-surgical. What to Expect

All procedures are done in a balanced and natural esthetic.

Consultation

The consultation focuses on creating a personalized treatment plan that addresses your goals and fulfills your expectations. I always ask my patients to list three things that they dislike about their nose. The purpose is to provide a framework and context for the procedure in an effort to fully address your expectations. We will devise a comprehensive plan that balances these objectives with your natural facial features in order to achieve a beautiful, long-lasting result. My goal is for you to have full confidence in your decision to make the change you have always imagined.

Surgery

Surgery is performed at an accredited surgery center in Newport Beach, California. Under the care of a Board Certified Anesthesiologist, general anesthesia (fully asleep) is used to ensure maximal comfort and safety.

Non-surgical contouring is performed in the office with topical anesthetic to ensure your complete comfort.

The incision for a surgical rhinoplasty is placed in the columella, or the bridge of skin beneath the nose. This area heals quite well and with time the incision becomes nearly imperceptible. Additional incisions are hidden inside the nose and generally heal without issue.

PRE-OPERATIVE GUIDANCE

1. Stop all Aspirin, Motrin, Aleve, Fish Oil, Supplements with Omega 3, Ginger, Garlic, Ginkgo Biloba and Turmeric – as these will thin the blood and complicate the surgery. You may take Tylenol as needed.
2. Medications – please notify us of any medications or supplements that you take regularly so that we may advise you appropriately.
3. Please fill all prescriptions prior to surgery so that they may be started immediately upon returning home.
4. Please no recreational drug use or smoking 4 weeks prior to and after the surgery.
5. Please have lab work completed 1-2 weeks prior to surgery.
6. Contact us immediately if you have any change in your health between your pre-operative visit and the surgery date.
7. Arrange for a driver to pick you up from the surgery center. Uber or Lyft are not acceptable. The driver does not have to remain onsite during surgery. they will be called when you are ready to be discharged from the recovery unit.
8. Make sure you have a responsible adult stay with you for the first 24-48 hours following surgery – this is for your safety.
9. DO NOT EAT OR DRINK AFTER MIDNIGHT THE NIGHT BEFORE SURGERY.

1. Take a shower using anti-bacterial soap – No lotions, hair products, powders, perfumes or makeup.
2. Wear loose comfortable clothing to the surgery center. We recommend wearing zip-up or button-up shirts and loose, comfortable pants.
3. Female patients will be given a urine pregnancy test at the surgery center.

Recovery

The procedure is well-tolerated and patients are discharged home the same day. Internal and external nasal splints are often worn for approximately 1 week to provide support during the initial healing phase. Patients can expect swelling that predictably dissipates with time. Bruising, if present, will resolve within one week. Patients typically stay out of work for about a week, however are permitted non-strenuous activity as soon as they feel ready.

Non-surgical rhinoplasty has obvious advantages in that patients experience no downtime. There is minimal initial swelling that resolves over a period of 2-3 days.

POST-OPERATIVE GUIDANCE

1. Prescriptions provided at your pre-operative appointment should be filled prior to surgery and started when you arrive home.
2. Please take a daily stool softener (e.g. Colace) or laxative (e.g. Senokot) while taking the pain medication. Stop if stools become loose.
3. Do not take Aspirin, Motrin, Aleve or herbal medications as these interfere with your blood’s ability to clot.
4. Bruising is a normal part of surgery. Over-the-counter Arnica can help expedite its resolution.
5. Continue to remain hydrated following the surgery.
6. Resume your regular diet, there are no restrictions. If you become nauseous, carbonated beverages and crackers may help settle the stomach. Zofran (Ondansetron) will be prescribed and can be taken as well.
7. Make sure you have a responsible adult stay with you for the first 24-48 hours following surgery – this is for your safety
8. Ambulate throughout the day to prevent blood clots in your legs.
9. No smoking – this inhibits wound healing, causing undesirable complications.
10. Driving – Do not drive or perform any tasks that require coordination or judgement for at least 72 hours after general anesthesia or while taking narcotic pain medication.
11. Do not lift anything heavier than a purse and no pushing, pulling, exercise or any activity that may raise your blood pressure until cleared.
12. Do NOT wear glasses of any kind for 4 weeks following surgery as this will impact the healing of the nose and final result.
13. Please advise our office with any travel plans or when you will be returning home if you are in town from out of the area.

1. Change the drip pad under your nose as often as needed. Expect to have bloody discharge for 2-5 days. DO NOT rub or blot your nose with a tissue.
2. Sleep on your back with your head slightly elevated at a 45o angle.
3. Cold compresses/ice packs help reduce the swelling, bruising, and pain when used in the first 48 hours after surgery. Twenty minutes on followed by twenty minutes off. Apply ice to the eyes, but do not get the cast wet or put pressure on it.
4. Carefully clean the edges of your nostrils with a Q-tip dipped in hydrogen peroxide to remove any dry blood. Clean the visible blue stitches with a Q-tip dipped in hydrogen peroxide 2-3 times daily. Once the area is clean, you may apply Mupirocin ointment to your stitches and around the rims of your nostrils.
5. Do not sniff or blow your nose. If you sneeze, open your mouth and sneeze through your mouth for the first two weeks.
6. While your cast is on, please do not take a shower, you may take a bath and wash your hair beauty salon style in the sink. The hair dryer must be kept at the lowest possible heat setting as your scalp may be partially numb. DO NOT GET THE CAST WET.
7. After the cast is removed, the nose can be washed gently with a bland soap and make up can be applied. Moisturizing cream can be applied if the nose is dry and scaly. Redness and bruising can be camouflaged by makeup once the dressings have been removed.
8. Refrain from eating hot soups or spicy foods. They will cause more drainage from your nose. Humidifiers, Chapstick and throat lozenges may help to ease dryness and sore throats caused by breathing through your mouth.
9. Please call the office if you experience fever, chills, increased pain, wound drainage or if you have any concerns. Our office hours are Monday-Thursday 9am-4pm and Friday 9am-12pm. To reach me after hours, please call the office and follow the prompts.

The cast will be removed 5 days after surgery. At this time, the nose will appear swollen and look turned up. The swelling will rapidly subside over the next few days as the nose begins to approximate its eventual shape; however, it usually takes about 1 year for the remaining 10 -15% of the swelling to disappear. This will not be noticed and will not detract from the appearance of the nose.

The cast removal does not hurt; however, some patients find there to be less discomfort if they take a pain pill before their first post-operative appointment. Someone must drive you to your post-operative appointment.

1. Please take a pain pill approximately 1 hour before your appointment time.
2. On the morning of your appointment, please take a shower making sure to get your cast soaking wet. If the cast falls off, do not worry. Don’t remove any of the tape underneath the cast. If you have an ear dressing, do not to get it wet when showering.
3. Before arriving to our office, please clean the outside of your nose thoroughly with a Q-tip and hydrogen peroxide but avoid applying any antibiotic ointment. (Mupirocin, Neosporin) to that area.

Results

Surgical results are long lasting provided trauma to the nose is avoided. Non-surgical rhinoplasty results are not permanent and require fine-tuning approximately every 12 months.

FAQs

After your cast is removed during the first post-operative visit, you will tape your nose at night for 3 weeks in an effort to control the swelling. Nevertheless, the last 10-15% will resolve with time beginning at the top of the nose and proceeding to the tip. The final definition in the tip will ultimately be evident at one year following surgery. It is important to know that this small amount of swelling will not detract from the appearance of your post-operative result.
